Accessibility and accommodation requests
A U.S. employer may have a person fill out an accommodation request form. Some may ask about a disability. Some may go so far as to require a doctor's signature. What are the best practices for accessibility paperwork in the U.S.?

@Meryl Evans - I had no idea that some employers require a doctor's signature! Just speaking from my personal perspective, I wonder how this would work for people who are part of the neurodivergent community. Thanks for sharing this, it's something I really want to look into further and I'm keen to hear other people's experiences as well as best practices!
